After Fredo asks to speak to Mike (and is denied), the sister asks to speak to Mike. The lawyer says "He's in the boathouse". Shortly afterwards, an explosion(?) or something occurs outside, as a large puff of white smoke rises. Was this a (hidden) sign of trouble to come?

You don't need any 'hidden signs'. You don't need any 'psychological hints'. You just have to look at Michael 'catching' Al Neri's eye. Al's attitude changes only a bit, but we all know that he has got the order to kill Fredo.
That is sinister!
