What was the hot water bottle, hugging the piano leg, zoom-in on the water leaking supposed to imply? Was that urine or water from the water bottle?

One of the most disturbing scenes in movie history. We all got hints about what the woman did to her and it came out at the end. I would have hated to play Hattie.

It wasn't a water bottle. It was a enema bag. The mother was filling it with cold water and inserting the tip of it into Sybil's bladder as a form of punishment/torture. When the child couldn't hold the fluid any longer the child urinated it out onto the floor, that's what you see on the floor. It is a form a torture to make someone hold their bladder when it is busting with fluid/urine.
