So when Dr. Elliot (Michael Caine) was checking his voice mail messages, had he actually called himself earlier using a weird "Bobbie" voice? With this scene, how could anyone ever guess the killer? Haha.
Also, in the split screen scene where he was listening to some recorded therapy sessions of Bobbie, was that just Dr. Elliot/Bobbie's character talking to himself into a recorder at his own therapy session? Or perhaps a recorded session with Dr. Levy? Again, it's pretty hard to guess who the killer is with this misdirection.

They seem to be implying that the killer's issue is not merely transgenderism but schizophrenia a split personality. The second psychiatrist says that "Bobbi" only "comes out" when Michael Caine's character is sexually aroused. It is also implied that when Michael Caine's character comes to the second psychiatrist that Caine doesn't realize he has actually been seeing the doctor as "Bobbi".

Yet people will say they knew who the killer was, without stopping to think about these things you say. What they didn't know for sure, was that Bobbie didn't even exist and was actually Elliott.
