The eery scene where the nurse enters the asylum ward where Caines characters is "sleeping" supposedly have taken his meds and dozed off. The first time you see this scene you nearly jump out of your skin (best watched while not holding anything scolding hot too). To see a beautiful woman going her duties with that musical score giving the impression something is about to happen but keeping you waiting is pretty much masterful suspense.
I think the scene is violent. I watched it first time with my wife and she was a little taken back that the scene did not end with the nurse succumbing to the trap set for her whereby she is strangled to death and laid across the bed but continued on with the crazies celebrating her death and then going into overdrive as she is slowly undressed and her underwear exposed.
Possibly the scene could have ended at the point she ceases to struggle and her face ceases to be etched in struggle as she is laid out by her killer. As a guy I would have thought that was enough too.
But I think the shock element was not added beyond that because all males are perverts and it would sell more tickets via guys who love to see women in films killed with maxium titillation as some feminist protesters of the day decided when deriding the file. Rather it seems to be included tp demonstrate just how tormented the Caine character is when the "male" side is aroused. Remembering she is bending over with her back to him when helping another sleeping inmate by reaching and putting their crutch away and the white dress stretching as she reaches far across the bed would likely expose her.
Beyond that its just a fact that crazies really are crazy when they want to be.
Probably the mostly shocking scene of the film. And the most erotic.
