I suppose they could have rejected him as a patient/client and turned him away. Technically they could have just done those steps directly to him or come up with other penalties to get him directly, pain, taking of his property car/house (vadalizing them or something). I guess find out what is important to you and go after that.

2nd offense Al think the shock might help make his daughter smarter.
3rd offense Peggy would be so sex craved that the guy they sent to rape her would end up running out of the house screaming.
4th offense Al would look at them and go "What do I got to live for?". He always asked God to take him now.

Yes I think this is why. Denatti says to Woods at one point that loving his wife and daughter is not in style these days but it makes it easy. This implies that he needs to use other methods with different people because they don't love their families.

Maybe each case is specifically tailored for each individual. Since these Quitters, Inc. people are so thorough (frigtheningly so) that they can hide out in your closet while you sleep, it shouldn't be too hard for them to come up with a plan for each person.
