Anjelica Huston, Tyne Daly and Kathleen Turner are the three best alternate casting choices imo. But Kathy Bates was the perfect choice in the end.

A more slight built actress would have haved to bulk up for it to work, like Rebecca DeMorney did in "Hand that Rocks the Cradle", DeMorney was too pretty to play Wilkes but perhaps Renne Russo could have?
Midler's cheeriness would have worked well for the 'sunny' side of Annie; she also does gloomy and manic really well, remember her in "Ruthless People"?

Maybe Conchata Ferrell or Edie McClurg, but she's more known for comedy. It's funny some have mentioned Tyne Daly, her partner from Cagney and Lacey, Sharon Gless played Annie in a play. She put on weight for it, which she never completely took off.

Stephen King's book described Annie as a big woman who was physically strong enough to lift Paul's entire body weight with no effort.
Billie Whitelaw doesn't fit that physical description.
