Maybe I missed something, but throughout the whole film I had no idea if it was a surreal reality or just a bizzare perception of a normal one.

IMO, it's both. in the movie we are able to see the "real" point of view, and the "insane/crazy" point of view, and it keeps alternating as the movie goes by.
