While I can't be certain, the first movie intro before the titles may have been
Frankenstein
(1931) when Edward van Sloan came from behind the curtain and warned the audience what they were going to see could be disturbing.
Dracula
, released a few months before closed with a curtain speech (again van Sloan) lifted from the original stage play. Apparently, it no longer exists.
Amadeus
had a particularly strong opening scene with the discovery of Salieri's attempted suicide.
And This, Too, Shall Pass Away

The first 10 minutes really tell the story of "Lonely Are The Brave" (also Kirk Douglas' favorite movie)… It starts with him in the open landscape desert, just he and his horse (Whiskey) enjoying nature.. Suddenly, he gets on his horse, and we see a whole new world.. Cars! Honking at him, and he's thinking, "Hey man, I've been on this horse before those cars" and it's a great display of two different worlds colliding. I think the prison break out was milked a bit too much, and kinda off the rails. I've seen it twice, but might wanna see it again.
