Here's my idea:
John Rambo is relaxing on his horse ranch in Arizona near the Mexican border. His old father has a Mexican foreman for the ranch. The foreman is good with horses but seems to be kind of a jerk and clashes with Rambo. The foreman is married to an attractive middle-aged Mexican lady (Salma Hayek)and has a teenaged son who looks up to Rambo but also wants to please his father. There is a strong attraction between Rambo and the wife, but Rambo does not act on it.
One day, Rambo's cantankerous old father finds out the foreman is letting Narco gangsters use an isolated part of the ranch to run guns, drugs and illegal aliens. The foreman kills Rambo's father and takes off for Mexico with the teenage son. Rambo brings the wife along to Mexico to show him where the Narco HQ is. Rambo has to get revenge on the Narcos while trying to rescue the woman's son.
After much bloodshed, the film ends with Rambo bringing back the woman and boy and settling down with them as a happy family on his ranch.
