Problematical cases are done by the oral surgeon.
I remember well, why I didn't like that idea.

I worry about pain too since painkillers don't do anything for me. But they're in the upper jaw so hopefully they'll be easier to remove.

I guess I have to just do it.
As said, I changed my mind about those beasts.
To make it worse, my family is notorious for barbed hook teeth. And I got them too.
When that dentist did'nt listen (he should've cut the tooth in half before pulling) it became 'real' fun. I didn't have pain (injection), pain came next day. But I remember well that he almost broke my jaw and I only thought, if it makes 'crack' I'll crack his nose. ☻
