I believe Gary is insane in the membrane, known as crazy-of-the-coconut in Europe. He was in an automobile accident where a part of his melon opened up and knowledge fell out, but he recovered to a degree by relying on different knowledge that remained in the bowl.
It's funny because sometimes he can just be looking off in to the distance like the lighthouse keeper is asleep, but then say something profound about love and space.

SANITY means health.
So whether is mental or physical it's correct. It's just that in English it became idiomatic.
