No, it's not just about aesthetic. It's a whole subculture. Goth isn't just cosplaying as vampires. It's not a very politically charged subculture, but it involves being accepting towards what most of society thinks of as freaks like trans people. Playing with gender and challenging gender norms is a part of it.

NH, it's an alternative culture, which means it won't necessarily be mainstream (subcultures often go in and out of fads, but they remain alive. Honestly prefer when they are "dead"), but it's not dead.
