Okay. I’m not that familiar with her, but I just looked her up, and as you say, the one in the middle is void of the arm tattoos.
So now I think I know which one is the real one. The one on the left has crooked doll fingers, so the logical conclusion is that it’s the one on the far right.

It was actually released last year but for some reason the Oreos weren't on sale until January of this year. Her vegas show and tour were put on hold due to the corona virus so she's going to start performing live again soon. In vegas there's a very pricy backstage package you can buy and actually meet her and take photos. I'm ever so tempted to do it or at least see the show the next time I'm there.

This is eerie lol. But IMO, it HAS to be the one in the middle. That’s the only one that appears to have an indent on the couch.
I can’t explain the lack of arm tattoos, as Nettie pointed out, but maybe they purposely hid it for this photo? Her arms are also in a weird orientation, which may mean she’s trying to hide her trumpet tattoo.

Did they model it on her real vagina
I wouldn't doubt it. According to the article, she actually met up with the people from Orient Industries, the people that make the doll, so that they could fashion the doll from her actual measurements.
