Jimmy Smits had great chemistry with Dennis Franz on "NYPD Blue" and they apprently got along just as well behind the scenes as well (which is more than Jimmy's predecessor can say), but David Milch's erratic screenwriting process (sometimes handing in scripts minutes before the cameras rolled) finally took his toll and he left early in the sixth season, with his character Detective Bobby Simone dying of a failed heart transplant.

The Wire - Stringer Bell
Boardwalk Empire - Jimmy Darmody
However these are ensemble shows so it is probably less dramatic than when a procedural kills off a lead investigator in some ways. Although for a procedural it is easier to replace a character with a similar new cast member.

When it's right for the story, I love it.
Lem
and
Shane
in The Shield fcking crushed me. Same with
Nate
in Six Feet Under, but I think that was more because of Michael C Hall's brilliant work at the funeral. -

Lt. Manny Quinlan in Harry O (1974-1976). Maybe it doesn't count, since he died in the second season, after the show had been moved from San Diego to Los Angeles, but he was a main character in season one.
In heaven everything is fine. -

As I remember, Quinlan died in season one. Harry O was only on two seasons, so Henry Darrow was killed midway through season one and replaced by Anthony Zerbe. I remember because Robert Dozier and Buck Houghton replaced Robert E. Thompson as producers, but season two was produced by Robert Dozier and Alex Beaton.

The lead detective from the first season of Law & Order was killed off in the second-season premiere, "Confession."
Robert Lansing's character was killed off at the beginning of season two of Twelve O'Clock High.
John McIntire's character was killed during the first season of The Naked City.
Boone was killed off in the first season finale of Earth: Final Conflict.
As I recall, Boone's replacement, Kinkaid, was killed at the end of season four of Earth: Final Conflict.
