          In 1993? It was depressing.
          I haven't seen it since.
You asked a pretty question; I've given you the ugly answer.

That is a good point to make to not want to see the film again, and while it went out of its way to be as careful as possible, (too careful), it lost some impact in the process. It has become rendered cheesy to my mind.

I think you are right. Thinking back on it, today, it would be a bit outdated.

Demme made it to assuage his guilt over some backlash he received over
                Silence Of The Lambs.
I really don't get why, as the film wasn't about homosexuality and if it then becomes a trans issue, a disturbed serial killer can come from any dynamic of representation. Part of the theme of the film was transformation, but on the dark side. It was a thriller after all and contextual.
                Philadelphia
was then made by straights to make something palatable for straights on the subject of AIDS and being gay. Now that was condescending.

"it's Beckett's friend approaching the bathroom to see why is is throwing up."
He's not throwing up. He has the runs. You can see him sitting on the toilet. And at the hospital he even has the conversation with the doctor there about where the doctor says "if the KS (Kaposi's Sarcoma) is causing the diarrhoea we need to know about that", and says they'll prep him for a colonoscopy.
