You have to be born and raised in Boston (or the surrounding burbs) to have one…or maybe just live here for a long enough period of time. I have an uncle who moved to Texas when he was in his twenties and he eventually lost his Boston accent and started talking with a Texan accent. He's in his nineties now and still lives in San Antonio.
