How can anyone think she was schizo? The ending literally reveals her as dead.

While the movie does reveal that she was dead, her experiences still resemble that of someone having a mental breakdown. The movie
Jacob's Ladder
(1990) is a similar film with a similar ending, yet the first time I saw it was in a course about mental illness.
