Please define each person's role in the attempted hit on Michael.

I have never been entirely sure as to what happened, except that Hyman Roth ordered the hit on Michael, and Fredo (perhaps inadvertently) had helped Ola and Roth. But what about these persons?
Johnny Ola: As Roth's messenger, did he direct the would-be killers? Bump them off when they failed? Or was he too obvious and not involved? Was Ola still at the compound or had he left earlier?
Fredo Corleone: OK, he gave up vital info. Did he also open the curtains? Did he kill the killers when they failed? Or was his role zero, or lesser?
Rocco Lampone: As head of security was he just not vigilant enough? Or something more?
Tom Hagen: Completely innocent, and the only one Michael trusted. Correct?
The hit men: Directly ordered by whom? Killed right afterwards by whom?
Anybody else?
