Debatable. Either way, she obviously had no care for his health and used his mental illness as a weapon against him while being purposefully ignorant of any consequences. Then she had zero remorse for his death. Which is why Ratched is one of the great movie villains

Nothing “professional” about weaponizing a vulnerable mental patient’s triggers against them. What she did would be enough to have her disbarred in a civilized society. A nurse should never cause a patient to harm themselves. Why else do you think she would bring up his mother, the source of all his trauma?

When a patient is under your care and suffering from a mental illness, if you then use that patient’s traumatic experiences against them and they start hitting themselves in front of you, because you were mad they got laid and had a human experience, then that is absolutely your fault as a healthcare provider. There’s no other reason why she would bring up Billy’s mother other than to cause mental/emotional harm to him. Ratched is truly a tyrant and terrible person.

I don’t even remember what led to that.. didn’t they break out of the institution which is clearly illegal and as authority it would require her to take action and give out some sort of consequence so it won’t happen again? She could have gotten in trouble if something bad happened to them while they were away since she’s technically responsible for their health and well-being. But someone killing themselves wouldn’t be her fault unless she told him to

They hadn’t tried to escape yet. When she showed up the patients had had a party and Billy had sex with a woman. She’s responsible for their health and well being, so then Ratched goes and mentally torments Billy for NO reason other than her own satisfaction and sadism. He then starts hitting himself and she doesn’t care. That is clearly cause and effect and she is responsible for Billy’s suicide, it doesn’t matter that she didn’t tell him to do it. She caused him to through her own negligence.
