I believe it was usually done with a bucket that one of the crew members would hold on the catwalk above the set. I know that whenever water or slime were used they would put plastic tarp over the floor to get a better sound effect from it.

one thing I also wondered is why did the kids often stick their heads up into the slime/water. I would've looked down so it didn't get in my eyes.

Roger Price, the show's director, told everyone to do this. I'm not sure what the reasoning was behind it; maybe just for dramatic effect.

actually the tipping your head up is done partly for comedic effect, and it will drip out of your eyes faster if your eyes are aomong the hither points of the face, simple gravity. if you turn your face down, like you tend to refelxively do, the stuff hitting the back of your head will slide through your hair and into your face, and your eyes. you will also notice that the ones who tipped up their faces would close their eyes and often go for the 'two handed wipe the eyes as soon as it stopped.
i know it's been a while since any comment on this idea, just thought i'd throw that out there.

Ahh, the physics of being slimed. But them looking up into the slime was just plain hilarious.
