This part has always confused me and I was wondering if someone could share their thoughts on it.
Dr Ruth seems like he's lost his mind and is just talking to himself about the Ripe Program, calling it cold and cruel, before Keller kills him. Has he just snapped?

I would say yes. I'm not entirely sure, but maybe he's put together what Revok's plan is.
