Was David Cronenberg joking with the audience by naming McGoohan's character Dr. Ruth after the famous sex therapist? Or was this just a coincidence if she had not become famous by that time?

He probably was. He's known for his fairly sardonic sense of humour. She was starting to get a fairly big name when the movie was made.
