If anyone is interested, I made my own chapter insert for
Bad Dreams
. I was also surprised Anchor Bay didn't include one. It's a shame because Anchor Bay and 20th Century Fox are the only companies that still produce chapter inserts.
Anyway, here are two links to images of the front and back of the chapter insert. Save it to your computer and open it in Paint. Make sure you have it so the image will print in the center of the paper (you may want to use a thicker and more durable paper). Print the front and then use the same paper and feed it into your printer so that the back image will line up and print out with the front image. Cut it out and then your done.
Note: You may not want to cut it out until it fully dries because it will curl and bend due to the ink drying
The images are the correct size so they will fit perfectly inside the DVD case. However, if for some reason you have problems, the exact dimensions are Width - 453, Height - 681 (in Paint)
