                  I'm thinking it's a play on words.
                  It means that John is difficult to kill, but it also describes him as a "diehard", a person who is strongly devoted to something.

                      A good movie title will have double meaning. In one sense it means die hard, as in devoted, or tenacious, and on the other hand it means he's hard to kill. It's a great title for a great movie.
