You have to think that in all their time together, the young couple had said to each other that they wanted children (without specifying a timeline). And they had been married four years already.

That's fine, but to be fair she decided all of that on her own, because she never talked to him about it. And she clearly knew that she was deceiving him, since she felt guilty about it.

I'm assuming you are not married or in a relationship, but the way it happens in the movie is the same thing that happens with most middle class couples when they decide to have children. The wife realises she wants to be a mother, with the first step going off the pill, they then start working towards having a child by seeing doctors, specialists etc. He obviously wanted to have a child too, or he wouldn't have gone to see a doctor and have a sperm count
