Not my top 5, but a few good ones I haven't seen mentioned a whole lot yet:
The Fountain (Clint Mansell, Kronos Quartet, Mogwai)
Friday Night Lights (Explosions in the Sky)
                Donnie Darko (Michael Andrews)
Mysterious Skin (Robin Guthrie & Harold Budd)
                Black Hawk Down (Hans Zimmer)
                28 Days Later (John Murphy)
Also, the best score I think I've ever heard is Bear McCreary's Battlestar Galactica Season 2 album. It's not technically a film score, I know, but it's still superb and something I highly recommend.

i guess you can make an arguement for either extreme
first when a score is as memorable as Glory that you find yourself here, or second, when a score is as ingrained into the film, like say Star Wars or The Godfather, that you don't even realize it. it is so at one with the action on screen that you forget about it by itself
if it wasn't for my horse, i wouldn't have spent that year in college

Great score here but while we're on the subject, best score ever is
                                        Body Heat
                                        Other notables:
King Kong (the Jessica Lange one)
                                        Out of Africa
You Only Live Twice (most of the Bond's have good scores)
                                        Chinatown
                                        All the Spaghetti Westerns
                                        The Mission
Jane Eyre (Susannah York and George c Scott)
                                        Taxi Driver
                                        Magnificent Seven
                                        The Third Man
