Sally Field is a thought, I think she could've nailed the personality. My only concern would be believing that she could be a dominating 'physical' presence as Kathy was 'cause she's such a tiny lady. She could've gained some weight and wore clothes with lots of padding or something, but still.
And obviously Meryl Streep, but she could play anybody.

(.. and Channing Tatum as Paul if they ever do a remake) 😉

Susan Sarandon sure has the potential to look psychotic with her large, expressive eyes but I think that, fundamentally, the actress would need to be slightly porculent. Just as Paul Sheldon's character needs to be of a slight build (more easily overpowered by his captor).
Oh and it's Annie Wilkes, not Wells (to the OP)
I think Tyne Daly sounds good - she's played such a character before. Glenn Close couldn't cut that sort of role - she's been there and done that in Fatal Attraction and is too slender IMHO. Kate Winslet, ironically, sounds a good choice as she can carry off all manner of characters (Think Heavenly Creatures, Titanic and Iris. She looked very different in all three movies and could easily gain a few pounds for a remake).

ellen burstyn, ABSOLUTELY. she wouldn't have been as convincing strength wise, as kathy bates, but she sure as hell can play crazy (requiem for a dream).
even though she would've been too young at the time, Winona Ryder comes to mind. The same goes for Shannen Doherty.
I think Oprah Winfrey could've pulled it off, she's scary enough, and would be believable in the fighting scenes since she's bigger.
Also Judi Dench and Marcia Gay-Hardin.

Nobody but Kathy Bates could be Annie. She's amazing; I'd love to have seen her when she played Jessie in "Night, Mother" on Broadway.

Its interesting that you mentioned Angelica Houston because she was nominated the same year for her brilliant work in "The Grifters." Even though i loved Kathy Bates in this role, i would have given the oscar to Houston. just my two cents.

                                  It's Annie Wilkes.
And I agree, Anjelica Huston would have been a good choice, and for some reason, I can see Rosie O' Donnell doing it. Don't be amused, I really can see her pulling it off. But Kathy Bates was by far the best choice.

Any good actress. There's no need for actress who's been doing similar movies before. A real good actress can be anything. Remember Kathrin Hepburn? And have you seen early Bette Davies movies, all her romantic roles or character that were making men lose their minds - and her horror roles, scary, creepy characters not like Jamie Lee Curtis who played in many horrors, but on the "sane" or "victim" side (wouldn't exclude her though!). So every really good actress can be imagined here.
Kathleen Turner, Sissy Spacek or Sally Field among already listed seem to be the best (though, unlike similar threads on different boards, most of offered actresses would be a good choice!). There is no reason to avoid any actress because she is making another type of roles, she is beautiful, older or younger good makeup, good director and good acting will make everything convincing. So why couldn't it have been done by Michelle Pfeiffer?
I'd add Mare Winningham to the list. It had been a whole decade after "Amber Waves", just finisher "Turner and Hooch" but have you ever seen her in Intruders? One of best female roles I've ever seen. As if you've taken a real farm woman and, just as curiosity, the character's name was Mary Wilkes.
I'm not certain about Geena Davis, but Candice Bergen would fit. Also, though not that famous, I'd add Joanna Gleason to the list. Shelley Duvall would be perfect, but hardly convincing as a person who might carry anybody through the deep snow.
                                    Anybody remembered Louise Fletcher?
And finally the one that could make you believe everything - Jessica Lange. It's hard to understand that she wasn't mentioned already.

Jennifer Tilly played essentially the same character in the movie "Cord." (aka "Hide and Seek")
Although many thought her performance was way too over the top, I liked it.
                                          So I nominate Jennifer Tilly.
