You have them held down by rope, chains, or soldiers.
And then you either quickly whack away at their chest, until the heart is "out" (gone).
Or slowly carve into their chest with a sharpened axe.

I would assume you use it to chop through the rib cage and then rip it out with your hand. It's what I'd do.
