If a film was made today then, yes, it would need to reference the profound mental illness and violence that is all too common with transsexuals, but back in 1990 we didn’t have an epidemic of psychotic trannies. I believe the film was true to what was understood about trannies at the time.

Most who began to identify as trans
(-sexual or -gender) in the past decade are not "real", either, in any psychological sense.
They are people motivated to "transform" by self-hatred. And in the age of woke, the establishment left and the medical establishment offered them a profound and collectively-sanctioned means of doing so.
