It was removed for Escape from New York.
He new that it would one day spawn a great sequal and wanted to have the one eye thing down by then.

And for the record. Kurt thought about removing his eye but due to his fear of Hospitals.the director insisted on him not doing it because he was afraid he would have a heart attack and die before they got to shoot this film!
