Please explain the original ending (the way it was originally shot)

I've read the board, and I've read where someone said Martha gets killed by a bolt gun to the head, but what's up with what looks like her being in a mental hospital in the trailer? What's with the body being taken down the stairs, etc., etc., etc.? In the trailer, it looks like Jessica stabs (or attempts to stab) Gwyneth. In essence, would someone please explain in detail the difference between what was filmed originally and what was actually available to us as the final version? Many thanks!!
