Tony dies.
Chase set up a clear pattern in that final scene:- Doorbell rings.
- Shot of Tony looking up.
- Shot of whatever Tony sees and hears (his POV).
This recurs several times as new characters enter the diner, until the final moments when it plays out like this: - Doorbell rings.
- Shot of Tony looking up.
- Blackness. Silence.
Now add that to the truckload of other clues - eg. the man in the grey jacket sneaking in and heading for the bathroom behind Tony, the discussion about the experience of being whacked with Bobby (this plays out exactly as they described so we experience the hit), the fact that Meadow will be entering the diner to see her dad’s head blown off, the way everything ends badly for all these criminals and Tony was on a death-slide to the inevitable for the final season (his ‘enlightenment’ was short-lived, he murdered Christopher, his therapist abandoned him etc etc)
Chase wanted to kill Tony but keep us talking for decades, and it worked.

The last shot is not from Tony’s POV, if it was we’d have a view of the door. It is just a shot of Tony that cuts to black. People assigning significance where there is none. The ending is spelled out that Tony was going to prison once Carlo testified against him.
The writers are basically on record saying the ending was meant to be ambiguous. Whether Tony lived or died is up for debate. Mafia hitmen don’t go sit in plain view in restaurants wearing suspicious goombah jackets. They burst in the front door wearing masks and baseball caps and gun them down at the table. Attacking from the bathroom is a retard move. The grey jacket man is just there to add ambiguity to the scene, Tony also eyes up the boyz II men gangbangers in the diner, it’s a callback to season 1. Member’s Only jacket guy is a callback to Eugene Pontecorvo.
If Gandolfini had lived long enough to do a sopranos movie, then the entire theory would be on the garbage can.
