Every time he got his "visions" or whatever you want to call it, he got incredibly thirsty.
I think the fact that he has his fridge stocked with nothing but orange juice furthers the idea that his mind is somewhat one-tracked at the time.
Example: all he can focus on is digging, it's taking over his life, all of the things he used to think were important aren't, just digging.

In the book, Tom points out to the reader, while dicussing his constant need for water after an "episode", that he remembers reading, or hearing, about how mediums get extremely thirsty after being used as a channel.

I thought it was because he was drinking orange juice the first time she appeared to him, sohe kept drinking it maybe hoping she would come back, like how he stayed on the couch forever since that's where she showed up. Just a thought.

I always took it to be that he was feeling what she felt. (i.e. choked & stuffed in a wall for a long time might make you thirsty)

I agree with most of these, but I think the obvious one is that orange juice can be used for a conduit w/ electrical current.
The partial electromagnetic properties of apparitions/spirits ( I'm applying what we know as of yet in the area of spirits and theories on how they are able to be seen or experienced within our 4 main dimensions) when applied to Kevin Bacons psychic connection to the victim, makes his choice of orange juice a subconscious choice to build a stronger connection. -

It helps with headaches and migraines. He was having constant headaches from the visions, and with a husband that has tbi in the military it is well known orange juice helps with headaches. Mushrooms help also.

I always thought it was because 'receiving' or whatever you want to call it gave him hangover like symptoms; Headache, thirsty etc
I don't know about anyone else, but when I have a hangover orange juice always makes me feel better. -

David Koepp mentions the thirst (and juice) in the commentary. He says it's just something that he added to leave the audience with unanswered questions about what it might be like to be haunted. Ergo; it's up to your interpretation of it.
