Most likely the towel Hannibal set on the exposed brain tissue would cause some sort of severe infection. It is also implied that Hannibal removed a large portion of a significant brain matter. The Krendler character looked unresponsive toward the end. It is pretty clear he was well on his way to death or coma.

The book takes the brain-eating scene to a far more gruesome place.
It's made clear to the reader that enough of his brain is taken out that he winds up a vegetable THEN they shoot him and kill him with a crossbow (at which point he's barely alive anyway.)
The end of the film kind of leaves his fate up in the air, to me anyway.
Wait a minute who am I here?
