The part that absolutely shocked me was the "your wife had a baby it's a girl" scene (I really didn't see that coming and it was pretty disturbing). The stair scene, of course, was creepy as well as Kana's descent up the stairs and the reveal to her mother.

Yeah, that part was really scary! Seeing Takeo like, hit the baby, on the phone booth. Oh my god, I wanna throw up! That scene was pretty scary.

The part with Kanna and the gaping hole where her jaw used to be would have scared me more if it looked more realistic. The computer effects are just too obvious, to the point of almost being laughable. But it was still a creepy scene.
I'd say the scariest part of this movie was probably Yuki's death. Just seeing Kayako's dark shape coming up to Yuki in the attic and seeing her there just inches from Yuki's face and knowing what's gonna happen, that's pretty creepy.
The Mizuho scene was pretty scary too.

Towards the beginning when Kobayashi first went into the house and is looking outside and you can see Kayako moving out for just a moment before it cuts to black.
When Kayako is crawling down the stairs towards Kobayashi - that was so terrifying, dude! I found myself trying to push myself away from the screen. It was kinda an odd scene, though, in that once she did get down and was hovering above his head and murmured his name, I suddenly found myself feeling bad for her rather than afraid. Heh.
When the brother and sister go into the house at the end and (my memory's kinda fuzzy) at some point before she goes into the eerie room, she sees Kayako and Kayako's arms are abnormally long. Creepy.
