I know that people complain that she is the manic pixie dream girl but I don't think she is. She's just a cool girl with stuff going on.
And to be totally honest if someone that looks like Sam and is that cool and likeable and fun, why the hell would Large (even in the state he is in) make his moves to shun her in the doctors office?
She is amazing and it makes no sense that he wouldn't be over the moon that she was showing interest in him.

Don't try to figure it out. It was just a badly written movie.

you ought to refrain from instructing people how to think or not think.
        it's rude and pretentious.
So what is it to you if he wants to contemplate why Zach's character is brushing off a cute, vivacious girl.
I bet you go around telling people how to feel, how they could've got a better deal on their T.V etc.
        Irritant.

You're right. This scene and thousands preceding it set up a false and terribly painful idea in guys heads that all they ever had to do was be a nice guy and magically they'd one day meet 'that' girl.
An entire industry is responsible for the devastating angst, confusion, anger and frustration of generations of young men who are lonely and sad because they were sold (purposefully or not) a great lie.

There is a vast difference between the way people insist on seeing the world and the way it actually is.

Well, when he was in the doctor's office, it was probably an awkward moment for him. Large was probably technically withdrawing from not taking his medication. He wasn't exactly a super outgoing kind of guy anyway. In the beginning of the film, Large just seemed so emotionally disconnected and mentally spaced. Which was because of all the medication he'd taken over the years. So I can't really blame him for being closed off towards Sam when he was at the doctor's office.
But the part I don't understand, is the ending. After all the time he ends up spending with Sam, and even after all his medication has most likely worn offhe actually considers leaving her and returning back to his old life. I just didn't understand that. You'd figure that Large would've realized what he was walking away from. But then again, in the end, he ends up staying with herso I guess you can say that everything worked out perfectly.
