I would add Fish Tank to this list, Sleeping Beauty (the Emily Browning version), and Kids. And if I dare straddle the boundary into documentary films; Terry Pratchett: Choosing to Die. Powerful stuff but definitely not lining up to watch that one again.

Irreversible is best seen once only, imo. Anything more is sadistic titillation without constructive purpose. This, on the other hand, I've seen twice, for the performances and outstanding direction. It's a very solid piece of drama.

I strongly disagree about Requiem. I must have seen that movie at least half a dozen times. One of my all time favorite movies.

I haven't really seen any other good movies that left me with a depressed feeling. I haven't even seen this movie.

Here's a short list of films, which I consider to be good cinema, but which are either so sad, violent, disturbing, etc., that I was only able to watch them once. They are all well worth watching, but I just can't watch any of them again. They are not listed in any particular order.
            Schindler's List
            Requiem For a Dream
            Brokeback Mountain
            Sophie's Choice
            Bent
            American Psycho
            Kids
Henry: Portrait of a Serial Killer
            Half Nelson
            The Parallex View
            American History X
            Chopper
            Looking For Mr. Goodbar
            Drugstore Cowboy
            My Own Private Idaho

MY ABSOLUT TOP: MARTYRS, GREAT film, however i never ever want to watch it again, left me with such a feeling of dispair, emptyness, anguish, depression. Definitely not for everyone.
On second place: IREEVERSIBLE, mentioned so many times in this thread.

                      Whoever mentioned
                      Fish Tank
made an excellent call. I was thoroughly entertained but I have zero desire to go back down that road again thanks.
Im shocked SHOCKED not ONE person has mentioned the movie
                      Inside
(although, granted, labeling it great might be a stretch. But its a solid film and Im sure some people think its great).
The movie is very disturbing and violent, sure. But beyond that, what makes it difficult to deal with is that the very thing you assume the director would NEVER dare actually put on film, he puts on film. The very direction you assume they would NEVER be evil enough to take us down is exactly what they do. And theres nothing you can do about it. You spend most of the film saying to yourself ok they arent possibly going to actually go there. No way. They wouldnt do that. Theyll come up with something shocking but manageable right? Right?? Theyll just tease us that the most horrific option has a possibility of happening so we are tense and anxious throughout the whole film then theyll do something else thats not nearly as AAHHH!!!! OH MY GOD NO!!!! MAKE IT STOP!! MAKE IT STOP!!
The film forces the viewer to be engaged in the unthinkable in a kind of
                      Clockwork Orange
way that Ive never experienced from any other film, even the most horrific violent gory depraved films imaginable. In other words, Ive seen films that were more gory and more violent but Ive never felt so psychologically helpless. Ive never felt like a movie was dangling me over the very pit of squirming tarantulas I was praying the whole time they wouldnt dare drop me in. And then they do they do
Using words to describe art is like using a screw driver to cut roast beef.

Requiem for a Dream is not that good that I'd watch it again. This movie, though, is.

Although this is a great film, it was an extremely awkward watch at times
