John tells Amanda that there's an envelope for her outside. She looks inside it and panics but we as the audience don't know the contents of the letter as of yet. From that moment on, Amanda's attitude and emotional state changes, especially towards Lynn. We find out in the later films that Hoffman wrote the letter stating that if Amanda doesn't kill Lynn he will tell John that she was responsible for Gideon's death (as we see in a flashback, along with Cecil). So the question as to why Amanda suddenly became so hostile towards Lynn, wanting her dead, then becomes clear. But here are my questions where the answers aren't so clear, or explained at all.

    1. How did Hoffman even know that Amanda was involved at the clinic?
    2. Amanda was threatened with information that John wasn't aware of. But John was the one who told her there was an envelope. Are we to presume he didn't know its contents? He would have at least known that it came from Hoffman seeing as he didn't write it himself. Would that have not raised questions between them? Surely Hoffman wouldn't want John to know about it as of yet
    3. If John DID know about the letter and what Amanda had done, is that why he kept saying that Lynn was more important to her than she knew? Was that part of her test, to let Lynn live, to show compassion, or to own up to honesty, even with the threat of being exposed?
    4. In one of the later films where we learn that Hoffman was there from the beginning, there is a flashback where The Rack was being set up. Amanda says to Hoffman "because I ain't going anywhere", to which Hoffman replies "You sure about that?". Was that to do with knowing that she will be tested, if my question 2 above is correct, and hoping she would fail?
5.In the flash back with Cecil in that chair with the knifes, why didn't he tell John that it was Amanda's idea to go to the clinic that night? He was clearly against doing it but Amanda kept pushing, so you'd think that would be the first thing he'd say, being in that predicament in the chair - "It wasn't my fault! A woman made me do it. I tried to persuade her not to!", "..what woman?"
    5. And finally, why did Hoffman even threaten Amanda with that anyway? Did he just want her gone? And if so, why?

It's never mentioned in the movies, but if you take the videogames as canon then the second videogame does clarify it. Haven't played it in a long time so I cant remember the specifics, I think he was investigating a drug cartel or something which eventually lead him to interrogating Cecil which was how he learned about the incident at the clinic.
      2. Amanda was threatened with information that John wasn't aware of. But John was the one who told her there was an envelope. Are we to presume he didn't know its contents? There's a few possibilities. Either John wrote his own envelope and Hoffman decided to sabotage Amanda's test by replacing it with the blackmail letter or John was aware of the letter and told Hoffman to write, just wanting it to be another obstacle in Amanda's test. Or the writer's completely forget about that one line that indicates John knows about the letter.
      4. In one of the later films where we learn that Hoffman was there from the beginning, there is a flashback where The Rack was being set up. VI is the only film where we get to see Hoffman and Amanda interact. It doesn't really go that deep into it but they both seemed to hate each other, which was enough of a reason for Hoffman to want her dead.

Thank you for your reply. Your answers have shed some light on those questions for me. I wasn't sure if I had missed something, but I guess a lot of the plot has been left unanswered. I mean, the plot though the whole 7 films is so detailed that its probably next to impossible to tie up every loose end. Still frustrating though lol.

Yeah, some movie story lines can be confusing.
