In terms of 'realistic' violence, this is definitely my #1 pick. I'm curious to see what other people think.

I just got this cheap on DVD at a record store because I heard great things. I'm curious to see how this will rank up with one of the most disturbing slashers I recently saw; Eden Lake. If you haven't seen it, do yourself a favor and watch it! Suspenseful throughout, and is not predictable! Don't watch the trailer either.

Yes, it's TOO real. It wasn't that scary to me at first but after I thought about it, I realized it may be the scariest movie ever since it could (and already has) happen in reality. I STILL think about it every time I'm home alone.

My reaction exactly - I've never seen such gore in a realistic way. You'll have movies like Dead Alive with more buckets of blood but that's done for comedic effect. This one was really serious. Easily 10X the gore of the Saw films. And just done very well.
