Unless told or shown otherwise with actual penis/vaginal penetration, ALWAYS assume the sex is .well, not fake, but not actual. I believe the word they use is "simulated".
There are rare exceptions (the wonderful 9 Songs and the less-wonderful Intimacy being two) but I woudl say 99% of mainstream movies cheat the audience this way.
