The puppets was just to enforce Sam's severe mental illness, like that is what he was seeing at that point in time, they were legit people trying to capture him though he just manifested them as muppet characters because he is insane.
And as far as the dick thing goes, he probably healed super fast because he is a supe? And by heal I dont mean his dick has grew back, I just mean the wound has healed and he aint in pain or at risk of infection or any other problems that would come in the immediate aftermath of having ones dick expload.
I'm sure he is angry and ****ed up mentally that he lost his dick at such a young age but what can he do? He cant make a complaint because he was trying to RAPE her essentially, and he cant really " get even " because she has a more deadly super power AND is friend with the most popular kids in school and way more powerful. I think he just accepts the consequences of trying to rape a woman who can control blood.
There is still an episode left so who knows? maybe he does have revenge in mind? -

I had just kind of assumed that that kind dick injury would have been the end of him through blood loss or whatever but I guess you are right and he healed up fast cos of his powers.
