I watched the movie again and found that I missed something. The father somehow gave herpes to her when she was a child. So it's just makeup!!!!

Mostly acne, though the cold sore on the bottom lip that shows up partway through is herpes.

It's acne except for the cold sore on her mouth. She has cold sores because her dad gave her mouth to mouth at a pool when she was little
