I'm convinced that the mother was genuine and that the reason the so-called twist was telegraphed so early is that we're MEANT to know. The story is about a troubled kid trying to get over the death of his twin brother.
However, the one thing I can't reconcile is why the mother faked the beauty spot / mole on her face. I don't understand why she would need to do that. -

Because she NEVER wanted to actually remove the mole/beauty spot, but it was a REQUIREMENT if she wanted to get the surgery done, otherwhise se would've kept it.
That's what she says, that the doctors said it would be best if it was removed.
That's why afterwards she had to draw it. Not just because she (probably) liked it, but because she was a tv personality and whatnot, everyone already knows what she looks like and is familiar with her face and all her traits, so if she were to suddenly be missing a beauty spot a lot of people would immediately be able to tell that she got some surgery done and that might not be such a good thing for her.
Fine, fine, I'll leave! But first I'm going to bother these peanuts! Hmm? Yes? Hmm? HMM?
