I've been like this for many moons
I used to take the pill for my skin but they won't give it to someone my age

Yes, you can get birth control or hormone meds after the age of 50 or 60. I just looked it up. You can consult with an online telehealth doctor, or you can see an endocrinologist or you can see a woman's health specialist who prescribes hormones for "off label use". I've done this myself to get on testosterone, which is a medication that women don't qualify for - it has to be done off-label at any age. You have to pay cash, but yes, you can get it.

I know you're frustrated because I remember that you've been talking about how well the pill worked for your acne for a long time and I remember you've been talking about how they won't prescribe them for you because of your age for a long time. It must be particularly frustrating because you've been on them and you know your body best and you know what works best for you. Out of curiosity, did they say that they wouldn't prescribe them for you because there would be negative consequences to your body, or did they just use the "you no longer need them" explanation? If it's exclusively because you can't get pregnant again BUT there are no concerns of negative repercussions to your health, then they're really just being lazy with your care. It would be so good if you had someone who would really come to your defense and go to bat for you, again, as long as the meds won't harm you.
Would you be willing to pay out of pocket if you could find a provider willing to work with you?

They don't want women past a certain age to take the pill because of a higher risk of cancer
Even if I did stop taking spironolactone, the Paxil alone makes me tired

Yeah, that makes sense. My cancer was estrogen-sensitive, so it makes sense they wouldn't want to mess around with your hormones if they don't have to.
Sucks about the Paxil. Have they tried you on another,
different
antidepressant at all? Maybe there's something different that might work that won't make you so tired. The side-effects on so many of these meds are just crazy af. You need meds to deal with the s/e and then more meds to deal with
those

I did try something different but I didn't like it
I just need to start taking vitamin B again
