I saw "A Serbian Film" and thought it was ridiculous, juvenile. The director was like "Hey, what's the worst thing ever? I know! Show a newborn being raped! That'll really seal the deal!" His laughable and nasty intentions came through a lot more than anything that took place on the screen.
Also, the movie was very dull.
My gorehound friend has seen just about everything. From ASF to Salo to Cannibal Holocaust to the Guinea Pig series, Necromantik, etc. And he said that the most disturbing film he'd ever seen was the 2006 German "Cannibal". I'm not a fan of the high shock value stuff ("Pink Flamingoes" has the saving grace of humor) and I never intend to watch it.

I never saw "Martyrs", but I did see "Inside" and "High Tension". Those were both very unpleasant, graphic films. And one viewing of each was enough for me.
Have you seen any of these French horror films? If so, what did you think?

I thought "Visitor Q" was both disturbing and intelligent, leaving the viewer with a feeling of hope at the end of the movie.
Takashi Miike is one of the most diverse directors I can think of.

Africa Addio is out of place since it is a documentary while the rest are fiction. From the list I've only seen that and Salo. Documentaries are more disturbing and shouldn't be compared with fiction in that way.

Andrey Iskanov's Visions of Suffering (Final Director's Cut) has a scene I'm pretty sure is real necrophilia
Emanulle in America has a horse love scene and the end is pretty brutal -

I did watch
Requiem For A Dream
on cable one evening and will never watch it again.
I've always maintained that
The Exorcist
is the most frightening movie I've ever seen, but it was just a devil possession fantasy (even if suggested by a real life incident).
Requiem For A Dream
is a very real possibility and on another level.
