        She knew who he was at the beginning. "Look, I know who you are. Why can't you just tell me?" That's why she was so upset in the Fortress of Solitude, even though she knows he refuses to tell her. It was essentially the equivalent of saying "I Love You". Hence her reaction at the end when he finally "revealed" his identity.

          But why does Martha call Lois after that meeting later in the movie, saying "Clark hasn't called." I thought Lois revealed to Martha that she knew his identity, hence they both knew Clark and Superman were one and the same.

            Isn't that when the Superman clone has shown up? That's what was proof to Lois that something was wrong. That Clark hadn't called Martha.

              She had known all along, watching it just now, she whispered the name "Clark" when Superman had "died". Him finally admitting it was probably a stage in the relationship, with the two sharing his biggest secret.

                My theory was that Lois was trying to figure out who Superman really was. She wasn't convinced by the name Kal-el, and when he finally admits to a biographical fact of Clark Kent's life, she accepts that as his true identity, as opposed to the "Clark Kent is the mask" theory.

                  Watch the movie again. It's obvious that Lois already knows that Superman is Clark even though he will not admit it to her until the movie's end. That is why she knows to go to Martha Kent's house in Smallville to talk about his death.
