His big screen career is over, unfortunately that ship has already sailed and sailed long ago. Had he gotten clean and sober and stayed clean in the early 2000's he probably would have been fine, but he just couldn't and the final nail in the coffin was when that sex tape with the hooker got leaked. Instantly killed any chance of making a slow comeback. He will only make direct to video movies/guest appearances on TV shows for the remainder of his career, which sucks because the guy could act and he was in some of my favorite movies of the 90's. Heat and Saving Private Ryan were two of my favorites.

I'm repeating myself, but if Robert Downey can make it all the way back, why not Sizemore? Don't know if he's still using. He came off so poorly on the Rehab show. They probably can't insure him for any halfway important movie. It's a damn shame.
The girlfriend he had on that Rehab show was horrible, a junkie hooker. Cringe. Not to mention Heidi Fleiss. Crazy. -

those are the things that i thought killed his career, drugs and that sex tape. my gosh, we'll never see him in a big movie ever again.
it's weird because for girls making a sex tape can be good kim kardashian and paris hilton come to mind but then again, that's different bc that's just reality tv and being a celebrity, not a star and not an actor.
