They essentially cease to exist as a person at that point so I'm leaning toward yes. We have the grace to put animals to sleep once they're past a certain point, yet would rather have humans wither away in agony and bewilderment until the bitter end

What is it, a dozen or so states where assisted suicde is legal now? If someone is of sound mind when diagnosed, yeah, they should have that right to make that decision. We do it for animals and it's considered humane.
