I think Joe Pesci is a pretty good looking guy. He's just doesn't have conventional-type good looks. And he's not fat. He's stocky in build but I noticed while watching today, he was actually in very good shape physically when he made this movie. Marisa Tomei is a beautiful woman but she is also playing an "out-of-work hairdresser" with a mid to lower class background. People think that if you are beautiful it's an automatic ticket to the big leagues romantically, but the truth is people of a given class tend to hang with people of the same class only. There's no guarantee that she would get some rich fabulous good looking guy to marry her no matter what she looks like. Most of the time I could see someone like Lisa making attempts to get a guy like that to marry her only to find out that most of them would only want to screw her on the side and marry a woman they would consider to be more refined.

I mostly agree with Zephyr's post. And to the OP, I know this may sound a little sexist/ignorant, but in my own personal experience and observation, I can say that it seems like women are less concerned with looks than men. I myself can think of several couples I personally know with an overweight/conventionally unattractive male and a slim female.
And when women are concerned with looks, it's rarely fitness/actual strength but usually rather presentation, clothes, and hygeine (again from what I've seen/experienced). Pesci's character had all of those in the manner that someone from Tomei's character's upbringing would've found attractive at the time.
If there's any disparity or problem with their pairing, it was in age and not attractiveness. Pesci was almost old enough to be her father or at least older brother, I think an attractive actress closer to his age would've been more believable.

One thing to remember is that you get the sense Vinny and Lisa have been together for a long time. I always assumed at least a decade.
There is that scene where they are at that cabin; Vinny is reading and studying for his court case, while Mona Lisa is pacing back and forth on the porch. This is the scene where she stomps her foot and complains about her biological clock.
In that scene, I believe she says to Vinny something along the lines of: "We agreed that we will get married when you win your fist case. Well, I have been waiting ten long years now."
Doesn't she say something like that? -

In all Honestly, Charisma trumps pure looks 9/10 times.
Vinny worked with her for years. Even if she dismissed him at first purely on looks, give a guy with his charisma enough time to work on her, and
As pointed out, by the time of the movie, they've been together 10 years, and as others have mentioned, the chemistry is what makes them a couple.
For me, the leaky tap scene sums everything up. He argues because that's his nature, she argues with him as he is challenging her, and they both get off on it.
