That last sex scene was rape since she did not consent.
It goes under the radar because it has Leo and Marty involved.

But yeah, I remember you even see a flash of a woman's vulva as she gets into bed with a man. Except for that infamous Sharon Stone scene in Basic Instinct, I don't think a vulva gets an R-rating.

It was originally NC-17, but Scorsese cut out some scenes with sex and drugs to get an R-rating.
